Clinic staff scheduling: 12-hour duties, night shifts and certificates
In private clinics, outpatient centres and medical centres work runs at two speeds: daytime outpatient hours, and night and weekend duty. Healthcare is one of the lines of work where the night-work limit allows an exception; but the 11-hour daily ceiling still applies. This page covers night duty for clinic staff employed under the Turkish Labour Law and how the system records it.
What makes staff tracking hard in a clinic?
What makes it hard is that very different working patterns share one roof. Secretaries and reception work outpatient hours; nurses, lab and imaging technicians take night and weekend duty; some physicians come on certain days of the week. When one spreadsheet tries to hold all of these, duty hours get mixed up.
Duty swaps are an ordinary part of clinic life: a nurse swaps a night with a colleague because a child is ill. If the swap is verbal, two things are lost — who actually worked that night, and whether enough rest was left between two duties. In healthcare the second is not only a legal problem but a patient-safety one.
The third issue is documents. Staff medical certificates, health-and-safety training and periodic medical checks expire; an expired document is usually noticed just before an inspection. A document sitting in a folder is not enough — someone has to know in advance that it is about to expire.
What limits the length of a duty in healthcare?
In short: for clinic staff under the Turkish Labour Law, daily working time may not exceed 11 hours; two posts may be arranged in 24 hours and night work may exceed 7.5 hours with the employee’s written consent. A 24-hour continuous duty does not fit this frame.
| Rule | Source | What it means in a clinic |
|---|---|---|
| Daily working time may not exceed 11 hours in any way | Working Time Regulation art. 4 | A 12-hour duty comes down to 11 hours net with a 1-hour break; a single 24-hour duty exceeds the limit |
| Two posts in 24 hours in healthcare | Shift Work Regulation art. 4(a) | Two 12-hour posts can be set up, day and night |
| Night work may exceed 7.5 hours with written consent | Labour Law No. 4857 art. 69 · Shift Work Regulation art. 7 | Written consent is taken from the night nurse |
| No overtime during night work | Labour Law art. 41 | Extending a night duty cannot be solved with overtime |
| At least 11 hours’ rest when posts change | Shift Work Regulation art. 9 | Staff leaving a night duty cannot be put on days the same morning |
Under art. 7 of the Shift Work Regulation the healthcare exception covers those working at the workplaces and branches of natural and legal persons providing or producing health services, and at social service institutions. Physicians may work under different arrangements (employment, self-employment, public service) subject to other legislation; we have not verified that, and this page covers only staff under the Labour Law.
How the system is set up in a clinic
Unit = department
Outpatients, laboratory, imaging, inpatient ward and reception are separate departments; the charge nurse sees and approves only their own unit’s staff.
Day and night duty
Two 12-hour shifts are defined with a 1-hour break. A shift whose net work exceeds 11 hours, or that does not meet the statutory minimum break, is not saved.
Duty swaps
A nurse wants to swap a duty with a colleague; first the colleague accepts, then the charge nurse approves. If 11 hours’ rest is not left between two shifts, the request cannot be opened.
Tablet at the staff entrance
Staff who do not carry a phone on the ward pick their name on the shared tablet at the staff entrance and clock in and out with a personal PIN.
Missing duty staff
Staff expected but not clocked in appear on the live board; the ward lead knows before handover time.
Expiring documents
For expiring documents such as health-and-safety training and medical checks, the manager is notified 30 days before expiry and on expiry.
Medical certificates encrypted
Documents such as medical certificates attached to leave are stored encrypted; only the person on leave and the approver can see them, and every view is logged.
Future-dated change
If someone’s duty pattern changes from a certain date, the plan is entered in advance and applied automatically on that day.
Example duty patterns and weekly hours
Two-post pattern: days 08:00–20:00, nights 20:00–08:00, each with a 1-hour break. Each post is 11 hours of net work.
| Pattern | Average net hours per week | Night post? | Watch |
|---|---|---|---|
| Days only 08:00–20:00, 4 posts a week | 44 hours | No — if the night period starts at 20:00 | Close to the limit; a fifth post exceeds 45 hours |
| Nights only 20:00–08:00, 3 posts a week | 33 hours | Yes — most work falls between 20:00 and 06:00 | Written consent; no overtime |
| 1 day · 1 night · 2 off (4-day cycle) | 38.5 hours | Half the cycle | At least 11 hours’ rest after a night |
| 5 posts a week | 55 hours | — | Over 45; if night posts are involved, the excess cannot be covered by overtime |
Under Labour Law art. 69, the night period starts at 20:00 at the latest, ends at 06:00 at the earliest and lasts at most 11 hours. Under art. 7 of the Shift Work Regulation, a post where more than half of working time falls in the night period is a night post.
What the system does NOT do in a clinic
It is a staff management system, not clinical software. We set out its limits up front.
- A single 24-hour duty cannot be defined. A shift with more than 11 hours of net work is not saved. A 24-hour duty has to be split into two posts, or your internal arrangement rebuilt with your adviser.
- No on-call type. A separate record type for being on call at home is not described for this system; time spent after being called in is recorded as a clock-in and clock-out.
- No patient data. The system is not connected to patient records, appointments or a hospital information system; no patient information should be entered into it.
- It does not store written night consent. Consent to night work beyond 7.5 hours stays in your personnel file; the system only warns.
- No physician performance or fee calculation. Pay by patient count or procedure is not calculated; no part of pay is calculated in the system.
Personal data: what to watch in a clinic
Employees’ health data is also among the data that moves around most in a healthcare business; keep it to a minimum in staff records.
- No biometrics — clock-in does not use fingerprints or face recognition. The Turkish Data Protection Board’s principle decision No. 2026/921 held that processing biometric data for attendance is unlawful even with explicit consent.
- Certificates only where needed — a medical certificate attached to leave is stored encrypted; only the person on leave and the approver can see it, and every view is logged.
- Location only during shifts — location is collected only while a shift is open and the app is open; location received outside working time is not stored.
- Privacy notice — a data processor agreement and an employee privacy notice template are ready in the panel; adapt them to your workplace.
Why should duty swaps go on record?
Duty swaps should go on record because the answer to who held the ward on a given night is only in the record. With a verbal swap the rota stays as it was; if something happens, someone else appears on the rota for the night than the person who worked it. In the system a swap request is opened, the colleague accepts, the charge nurse approves and both shifts trade places for that day; the next day everyone returns to their own pattern.
The record’s second job is protecting rest. Putting a nurse who has just come off nights back on duty the same evening happens easily in a well-meant swap. The system measures the rest between two shifts in the swap request and will not open it if 11 hours are not left, and tells the requester why.
The third is month end. Night work, weekly rest days and overtime appear as separate items on the timesheet; the approved month is locked and goes to your accountant as a payroll input package. Who worked which night, swaps included, sits in a single record.

When clinics ask
Our nurses do 24-hour duties; can we define that in the system?
Not as a single 24-hour shift. For staff under the Labour Law, daily working time may not exceed 11 hours in any way (Working Time Regulation art. 4), and the system does not save a shift with more than 11 hours of net work. Because two posts may be arranged in 24 hours in healthcare, the duty is split into two 12-hour posts.
Can a nurse coming off a night duty work the same afternoon?
At least 11 hours’ uninterrupted rest is required when posts change (Shift Work Regulation art. 9). Someone who leaves at 08:00 can start a new post at 19:00 at the earliest. The system measures this in shift swap requests and future-dated changes; if it is not enough, the request cannot be opened and the earliest suitable day is suggested.
What is needed for a night nurse’s 12-hour duty?
Because night work exceeds 7.5 hours, the employee’s written consent is required; healthcare is among the lines of work where this exception applies (Labour Law No. 4857 art. 69). Consent is personal, taken from each employee separately and kept in the personnel file. The system saves the shift with a warning and does not hold the consent.
Can we rotate the same staff between daytime outpatient hours and night duty?
Yes; a rotating plan assigns day and night shifts in turn. Art. 8 of the Shift Work Regulation provides that someone who works nights for at most one working week moves to days the next working week, or a two-week rotation where the work allows. The 11-hour rest is measured at every changeover; build the plan around both rules.
Can we track our doctors’ clock-ins too?
Technically every staff record uses the same clock-in methods. But the type of relationship a physician has with your workplace (employment contract, self-employment, partnership) changes which rules apply, and we have not verified that on this page. For a physician on an employment contract the frame on this page applies; for others, ask your adviser.
Who can see an employee’s medical certificate?
A medical certificate attached to a leave request is stored encrypted; only the person on leave and the approving manager can see it, and every view is logged. If you want nobody beyond the unit head to see it, you set up the approval chain accordingly.
